Criteria

• Please note – self-referral is for physiotherapy only.
• Please note – If you are under 16, have recently had surgery or have multiple joint or muscle
  pains please do not complete the form but seek assistance from your GP.
• In some cases, you may be required to see your GP for further assessment prior to being referred into the service, in which case we will contact you and let you know.

Your referral, outlining the problems you have been having, will be assessed and triaged by our experienced clinical team. You will then be offered a convenient appointment at one of our locations.

Please see your GP first if you have:

• Existing diagnosis of Cancer.
• Severe Low Back Pain with night sweats, fever, sickness.
• Unexplained weight loss.
• Severe constant night pain.
• Severe pain with history of fatigue.
• Hot or swollen joints.
• Generally unwell.
• A problem that is effecting more than one body part.

• Assessment and diagnosis of musculoskeletal conditions.
• A plan to inform, educate and deliver a programme suited to your individual needs.
• Treatment for patients which may involve advice/education, exercise therapy and/or hands on therapy with emphasis on exercise and adopting a healthy lifestyle.

Appointments and Classes

• Physiotherapy 1-1 appointments.
• Physiotherapy telephone consultations.
• Classes including: OA knee/lower limb class, Functional mat based back class, MSK functional Rehab class, Shoulder class, BeST back class and Back School.

• To assess, diagnose and tailor management plans.
• To assess complex musculoskeletal problems.
• To assess patients for which conservative management has not worked.
• To assess and direct referral for further imaging investigations (MRI, US, X-ray, NCS) or bloods as appropriate.
• To assess patients that may be suitable for and deliver injection therapy.
• To screen and complete onward referrals for patients who may need secondary care intervention or alternate service management.
